This monster is balanced to fit in a high level of balance game. I have taken the liberty of not using the standard D&D rules for determining various statistics for this monster. As a result, it may seem a bit stronger (or weaker) than some SRD monsters of the same CR and that is fine. I think it will pose an appropriate challenge for characters of its CR.

Disguise self (Sp) | Annis hags can use disguise self as a spell-like ability, at will. This spell also allows them to specify a disguised alignment and disguises their voice. Their caster level is 6, Will save DC 19 if interacted with. |
Rend (Ex) | When an annis hits with both claw attacks, it rends the flesh for an additional 2d6 + 10 damage. |
Improved grab (Ex) | To use this ability, an annis must hit a Large or smaller opponent with a claw attack. It can then attempt to start a grapple as a free action without provoking an attack of opportunity.
An annis can attack a grappled foe with both claws at no penalty. |
Bubble bubble toil and trouble (Su) | Annis hags can create negative versions of potions. They can brew inflict potions (up to Inflict Serious Wounds) which identify as the cure version unless Legend Lore is used or a successful Craft alchemy check opposed by the hag's roll (determined when she brews the potion). A typical hag will have at least one potion already brewed. They can use this for their tricks after gaining a party's trust, but make sure to give some real treasure too. |
Black magic (Su) | An annis can curse its foes with a terrible sickness. As a move action, the annis can point at a target within 30 ft. and utter a dark curse. The creature must make a Fortitude save DC 19 or immediately contract cackle fever. |
Practitioners of witchcraft with a taste for the flesh of sentient beings, annis hags are tricky giants which lure people into a false sense of security and trust before weakening them and devouring them.
Strategies and Tactics[edit]
In combat, an annis will try to get a grapple by hitting with claw attacks and rip its target to shreds while grappling. Their sneakiness comes by how they manage to get close without hostility.
Annis hags have been known to use the following techniques to lure in do-gooders:
- Transform into a young looking female giant and cry while off the side of the path. The annis will tell a sob story (bluffing all the way) and lure the group to "help her" into some remote area. She may have friends waiting or she might try to separate one of the group. Either way, she will eventually get close and strike.
- Transform into a crippled goliath with an exposed severely burnt leg, using makeshift crutches. She will get sympathy from the group and ask them to spare any medical supplies for her group that was ambushed by a dragon/salamanders/some sort of fire beast. She leads them back to the "goliath camp", where the coven of hags gets to pounce on them.
- Transform into a half-giant "hero" fighting another transformed hag looking like an ogre. They will be "fighting" off the side of the road with the half giant saying lame things like "back foul monster!", etc. When the group comes to help fight the ogre, they are suddenly assaulted by both hags.
