This will leave a scar.
Make a melee attack. If you hit, the target must make a Fortitude save or have a gaping wound open up on their body which wracks the body with pain for 1 round/level, or until they are brought back to full health. This causes a -5 penalty on attacks, saves, and skill checks.
In addition the wound bleeds profusely, dealing your initiator level in damage at the start of its turn each round.
On a successful save you still deal damage, and the duration is only 1 round.
