Face of the Obyrith

For the truly insane or desperate, this graft can replace their entire face with that of an ekolid. Aside from being merely terrifying, it also grants the user the form of madness common to ekolids.

Any non-demon creature meeting the user's gaze within 60ft must make a Will save (DC 10 + 1/2 the user's character level + the user's Charisma modifier). If this is failed, the creature begins to suffer the hallucination of bugs crawling over and under their skin, incurring a -2 penalty on all attack rolls, damage rolls, skill checks, ability checks and saving throws and becoming unable to perform any act that requires concentration without a successful Will save (using the same DC). This affliction is permanent unless removed by a lesser restoration, heal, miracle, or wish spell, but a creature that saves successfully is immune to this ability for 1 hour.

The user can spend a standard action to gaze actively at a single creature within 60ft to force a save against the above ability, but only for that creature.
