Maybe it's just me, but a DC 20 check for a 2nd level spell seems really intense. Why not follow the spell save formula and at worst provide disadvantage. Also, does the creature still get incapacitated if the cyst is removed with magic? I really like that it's a super story board spell, but it seems really bossy for a 2nd level spell. - This spell was intended as a compromise between Hex and Bestow Curse; so i'm surprised you consider it fiercer than Curse. I'm not sure what you mean by "bossy", the spell just lays out all the various methods by which it can be cured after the victim has failed their initial save.
